As outlined in NESD AP 605 – Home-Based Education and the Saskatchewan Ministry of Education’s Home-Based Education: Policy and Procedures Manual, families are responsible for submitting a grade-appropriate education plan for each home-based learner.
This report should provide your reasoning and philosophical approach to home-based education and a minimum of 3 broad goals for each of Language Arts, Mathematics, Science, and Social Studies, and describe the means of assessing and recording of educational progress in each of these broad goals. The year end report provides evidence of learning and growth in relation to the goals identified in the student’s education plan.
For families with more than one child registering in home-based education, a separate Education Plan is required for each student. If you choose to send information in one combined document, each child’s plan must be grade-appropriate, clearly separated, and individually described.

This report should show evidence of learning and growth in relation to the goals identified in the student’s registration and education plan.
The Year-End Report helps NESD meet its responsibility to monitor home-based education programs by reviewing student progress in relation to the written education plan, as well as the student’s age and ability. It should include a summary of learning growth and may include samples of student work, portfolio evidence, assessment information, or other documentation that shows progress toward the goals you submitted.
For families with more than one child registered in home-based education, a separate Year-End Report is required for each student. If you choose to send information in one combined document, each child’s learning must be clearly separated and individually described. The report should celebrate each child’s learning and show evidence of their individual growth based on their own education plan and goals.
